c44c0067eb Fix types for PHP 7; fix argument parsing 
PHP 7 uses 64-bit zend_long and string lengths are size_t.

The preferred protocol was getting overwritten with a default 0.
Only use this argument if more than 2 arguments were specified.

8db0824d92 Initial commit of the PC/SC (pcsc) extension. There is some stuff that needs to be romoved like the build.sh script and the subfolders tmp/ and php/. 
I think the .m4 file has some bugs because it fails to build without my weird build.sh script. I will fix that ASAP.
